Where it usually goes wrong
The dangerous defect in a financial system is rarely a crash. It is a number that is quietly wrong and keeps being wrong, because nothing in the pipeline was built to disagree with itself. Reconciliation added late finds discrepancies it cannot explain.
How we approach it
We build the disagreement in first: independent paths to the same figure, and a loud failure when they diverge. It costs more at the start, and it is the reason a discrepancy is a ticket rather than an incident.
